Prevention & Intervention Services
The Prevent Department assists the CUA Case Manager with identifying supports and resources, as well as advocating for families who are in crisis or transition. The Prevention Department also works with families to ensure safety and promote well-being. Utilizing a strength-based approach to parent engagement; prevention team members supervise visits, assist with medical and behavioral health needs, actively engage all caregivers (including fathers), recruit potential foster homes, engage the at-risk teen population, and host a variety of events and activities, such as Parent Cafés, that connect families to resource and develop social skills.
Obtains medical history and basic health Information for all children and youth involved with the CUA. Ensuring that the children and youth have their medical documentation and make certain that medical treatment plans are being followed.
Assist CUA Case Managers with youth on their caseload. Teaching youth to understand and perform life skills related to self-care, housing, money management, work and study skills, career and education planning, relationship, communication, and daily living skills. Collaborate with the Youth Mentors to develop/host events that are youth centered.
Coordinates with the CUA Case Manager to provide an appropriate visitation strategy that ensures the safety of the children and youth. Providing supervision and hands-on coaching and modeling during family visits.
Engages parents, providing direct support, identifying concrete supports and services, guidance, and mentoring to our clients who may struggle understanding and/or navigating experiences being involved with the system.
Engages youth on a daily basis, providing emotional support, mentorship, guidance, connecting youth with beneficial resources, identifying and working towards goals to assist them in becoming successful members of society.
Promotes and Integrates the Strengthening Families framework within CUA practice and culture-as well as the larger community via a collaborative approach. Engages parents and the community at large via Parent Cafés, community engagement events, and other opportunities to educate caregivers and family members about how enhancing Protectives Factors can improve their family dynamic.
Engages and recruit perspective resource parents and collaborate with Resource Home Recruiters in the CUA region via community events. Serve as a liaison and support to resource families, offering assistance prior to, during and after certification process is complete.
Works to build and maintain collaborative relationships and partnerships with various community stakeholders. Promoting community engagement activities and events both internally and externally and finding resources in the community to benefit families.
Case Management
Each family referred to CCS has a single CUA Case Manager who works on a single case plan that is child centered and family focused. That Case Manager receives supports from an entire team to develop quality, efficient, comprehensive case management.
CUA Case Managers visit resource (foster) home, other out of home placements, and the homes of biological parents to work towards identified goals with the objective of reunifying children with their families and/or finding a safe permanent option.
Community Engagement
Each family referred to CCS has a single CUA Case Manager who works on a single case plan that is child centered and family focused. That Case Manager receives supports from an entire team to develop quality, efficient, comprehensive case management.
CUA Case Managers visit resource (foster) home, other out of home placements, and the homes of biological parents to work towards identified goals with the objective of reunifying children with their families and/or finding a safe permanent option.
Fatherhood Initiative
Fatherhood Initiative Coordinator engages, primarily the fathers and/or father figures to identify, address, and overcome challenges. Assist fathers in obtaining employment, building relationships, positive decision making, financial literacy, educational resources. Facilitating parent groups, support groups and workshops to educate and empower male caregivers.
